Aftercare — continuing care for the long arc

Recovery is not finished when a program ends. Aftercare provides weekly group and periodic clinical check-ins to maintain the gains you made and catch slipping early.

Key takeaways

  • Aftercare is weekly group (60–90 min) plus a monthly 1:1 check-in with a clinician you already know.
  • Most aftercare stays run 3–12 months; some run longer.
  • If you need to step back up to IOP or PHP, the door is open — no new intake.
  • Continuing care substantially reduces relapse and re-hospitalization risk versus stopping treatment at program end.

Who aftercare is for

  • You've completed PHP or IOP with us and want a structured next step rather than going straight to once-a-week therapy with a separate provider.
  • You're already established with an outside therapist or psychiatrist and want a group + check-in cadence on top of that.
  • You're in early recovery from substance use and benefit from a clinical group that knows the language.

What aftercare looks like

  • Weekly group, 60–90 minutes. Process-oriented, facilitated by a clinician you know from your earlier program.
  • Monthly clinical check-in. 30 minutes 1:1 with a therapist to review goals, adjust plan, flag emerging issues.
  • As-needed psychiatry. If you have ongoing medication management with us, that continues at whatever cadence you and your prescriber agreed.
  • Open door. If you need to step back up to IOP or PHP, the door is open — no new intake.

Length of stay

Aftercare is designed to run as long as it's useful. Most people stay 3–12 months. Some stay longer. We re-evaluate every 90 days with you and step down when it's the right time.[1]
