Admissions
Start your intake
If you'd rather begin in writing than on the phone, you're in the right place. Share a few details below β what brings you here and how to reach you β and a real member of our admissions team picks it up from there. No bots, no forms-limbo, no insurance acrobatics up front.
Start your clinical intake
Tell us a little about what brings you here. A member of our admissions team β a real person β reviews every intake and follows up directly, usually within one business day.
Your intake is in.
Thank you for trusting us with this. A member of our admissions team will reach out directly, usually within one business day. If you need to speak with someone right now, call us at (949) 735-5705. If this is an emergency, call 911 or 988.

What happens after you submit
- A person reviews it. Someone from admissions reads your intake β not an autoresponder. We typically follow up within one business day.
- A 20-minute conversation. We listen first: what brought you here, what's been tried, what you're hoping for. No diagnostic interrogation β just a conversation.
- Insurance verification. If you have insurance, we verify benefits for you β you don't need to call your carrier. See insurance details.
- Clinical assessment and start. A clinician confirms the right level of care, and most people start within 1β3 business days. See what to expect on day one.
Your privacy
The intake form above is the only form on this site built to receive health information. It is transmitted over an encrypted, HIPAA-covered connection and handled separately from our general website contact forms. Your records and the fact of your treatment are confidential under HIPAA and (for substance use records) 42 CFR Part 2 β we only release information to a third party with your written consent.
For the full picture, read our Notice of Privacy Practices.
This form is not for emergencies
If you or someone you're with is in danger, call 911. For a mental-health or suicidal crisis, call or text 988 (the Suicide & Crisis Lifeline), available 24/7. We monitor intakes during business hours only.
